    Background and aimsSecondary vasculitis of the central nervous system is a rare manifestation of neuroborreliosis. We present the case report of a patient with a fatal course of this disease.MethodsThe case report describes the course of a rare disease. The case concerns a woman, 49 years old. The patient was admitted to the infectious disease department with a diagnosis of suspected neuroborreliosis. The complaints reported were headache, dizziness, diplopia, gait disturbances and lower limbs paresthesia.ResultsThe patient was treated with antibiotics. At clinical progression, CTA showed multiple stenoses and occlusions of intracranial cerebral arteries (occlusions a. basilaris, both vertebral arteries, both anterior cerebral arteries). The picture corresponded to vasculitic changes. On further investigation, no other cause of vasculitic changes was found. Despite significant immunomodulatory therapy (corticoids and cyclophosphamide), there were extensive ischemic changes in the brain tissue with subsequent edema. Anti-edema treatment was administered and a decompressive craniectomy was performed. Edema and ischemic changes continued to progress. Despite maximum therapy, the patient died.ConclusionsSecondary vasculitis of the central nervous system caused by neuroborreliosis is a rare disease. Early initiation of immunomodulation therapy is the only possible procedure. Despite maximum therapy, however, the disease can have a fatal course.
