Original language description
Clostridial infections remain a rare entity in abdominal surgery but with high overall mortality rate. We faced the case of 75-year-old man operated on for cholecystolithiasis, where open cholecystectomy was performed. Within 48 h after the operation, clostridial sepsis and myonecrosis of an abdominal wall developed. The cause of the endogenous clostridial infection was the accesory bile duct found during the revision. Microscopically gram-positive rods and gram-positive cocci were seen, and colonies ofmainly Clostridium perfringens were found in cultivations! An agressive antibiotic therapy in intensive care unit was started, and the patient had to undergo five surgical debridements and necrectomies in general anaesthesia.
