Introduction: Sentinellymph node (SLN) biopsy and subsequent clinical assessment show different results in some patients with malignant melanoma (MM). The incidence of a false negative SLN is less than 2% in a case of comparison of SLN status with a histological finding in the remaining lymph nodes in the Iymphatic basin. However, the incidence of lymphatic metastases in the adjacent lymphatic basins varies in patients with negative SLN from4% to 27% duringthe follow-up (Mariani G et a1.JNucl Med 2002;43: 811-827). The aim of the study was to evaluate this parameter in a large group ofpatients. Methods: 170 consecutive patients with MM underwent SLN biopsy and subsequently were followed at the Dept. of Oncology. 2 patients were excluded from the studydue to the local recurrence with secondary lymph node metastasis, therefore a total of 168 patients were studied. Loca1ization of MMs: 91 trunk, 67 extremities, 10 head and neck. TNM classification: 48 pTI, 65 pT2, 32 pT3, 19 pT4, 4 pTiS

Spolehlivost biopsie sentinelových uzlin u maligního melanomu

Czech description

Cílem studie bylo pomocí klinického sledování zhodnotit spolehlivost biopsie sentinelových lymfatických uzlin (SLU) při detekci uzlinových metastáz u pacientů (pac.) s maligním melanomem (MM). Celkem byla SLU biopsie provedena u 170 pac. s MM. 2 pacientis lokální recidivou MM byli vyloučeni ze studie pro možnost vzniku uzlinové metastázy na základě recidivy. Celkem hodnoceno 168 osob. Lokalizace MM: 91 trup, 67 končetiny, 10 hlava a krk. TNM: 48 pT1, 65 pT2, 32 pT3, 19 pT4, 4 pTiS. Střední doba sledování 2,6 roku (0,5-5,7, medián 2,4 roku). SLU nalezeny u všech pac. ? 100% úspěšnost detekce SLU. Metastázy v SLU detekovány u 25/168 pac. (1/48 pT1, 6/65 pT2, 8/32 pT3, 10/19 pT4). Následné klinické sledování zjistilo lymfatické metastázy u dalších 4 pac.(1 pT2, 1pT3, 2 pT4), z toho ve 3 případech šlo o MM na distálních partiích dolních končetin.
