A three year old, 920g, intact female guinea pig was presented to the authors? clinic with a four month history of non-pruritic hair loss on the ventral abdomen. The physical examination revealed: good body condition, flank and ventral abdominal alopecia, mucoid vulvar discharge and abdominal distension. Bilateral rounded structures just caudal to the kidneys, as well as structures consistent with enlarged uterine horns, were detected on abdominal palpation. Moderate hyperglycaemia (8.5 mmol/L; 3.3?6.9 mmol/L), and markedly elevated plasma oestradiol levels (241 pmol/L; reference range up to 110.13 pmol/L) were found on analysis of plasma. Abdominal ultrasound showed the presence of cystic ovaries with thickened uterine horns and with multiple circular hypoechoic and anechoic structures in the uterine wall. After stabilization, the patient underwent ovariohysterectomy. Recovery was uneventful. On the gross pathology, a piece of hay was found in the left uterine horn. A final diagnosis based on these examinations, along with histological and culture evidence, indicated the presence of hormonally active ovarian follicular cysts, cystic endometrial hyperplasia (CEH) and purulent bacterial endometritis, caused by Escherichia coli, Fusobacterium nucleatum and Arthrobacter spp. CEH is infrequently reported in guinea pigs and to the authors? knowledge, this is the first report of a case in a guinea pig of CEH-bacterial endometritis complex associated with the migration of a foreign body into a uterine horn.
