    Rapid typing of Staphylococcus aureus using high-resolution melting analysis – effective tool for hospital epidemiology

    Background: Staphylococcus aureus is a major cause of hospital-acquired infections and fast molecular typing methods constitute an essential tool for effective surveillance, outbreak investigation and infection control. The aim of this study was to implement a time-saving and cost-effective approach for routine S. aureus typing. Materials/methods: We combined two high-resolution melting (HRM) based typing methods: Minim Typing and HRM-Spa typing (using genes derived from MLST and Spa typing). Minim typing was performed as a six fragment HRM assay and the final Melt Type (MelT) was defined as a combination of six HRM alleles. Spa typing was performed as one fragment HRM assay and the final Spa type was identified through the characteristic HRM curve profile. The typing scheme was tested on 259 clinical S. aureus isolates, 154 methicillin-resistant S. aureus (MRSA) and 105 methicillin-sensitive S. aureus (MSSA) collected between 6/2016 and 10/2017. Results: All the isolates were tested with Minim typing and we obtained 21 different MelTs in total. The majority of MRSA isolates was identified as MelT72 (62%) and most of MSSA isolates were identified as MelT7 (60%). These MelTs were determined as ST225 and ST8 respectively, by MLST. MelTs represented by at least two isolates were subsequently analysed with HRM-Spa typing. In this way, 250 isolates from 12 different MelTs were divided into 51 distinct Spa types. Isolates within one MelT were sorted into two to twelve Spa types. The final results are obtained in 4 hours and analysis costs 9 - 16 EUR. Conclusions: We introduced a rapid, reproducible and cost-effective typing scheme for both MRSA and MSSA isolates. HRM-Spa typing represents a useful typing tool in connection with Minim Typing, although it produces a huge amount of indistinguishable curves without Minim Typing (currently 17000 Spa types). Compared to sequenced-based techniques, this approach is suitable for large sample set screening and can be easily applied in routine clinical practice. In addition, the assay provides portable data that can be directly related to MLST and Spa typing data.
