Abstract—Interictal epileptiform discharges (IED) are an electrophysiological hallmark of epilepsy, and their occurrence characterizes brain tissue that is connected to the epileptic network of focal epilepsy - irritative zone (IZ). IZ is composed of partially overlapped sub-regions generating IED independently. Various ways of spreading IEDs complicate localization of IED sources resulting in sub-regional fragmentation or insufficient decomposition using signal analysis of stereo-EEG monitoring for surgical treatment of epilepsy. We localized the sources of IED for each sub-region of highly fragmented IZ resulting from the previously published algorithm. To identify and merge sub-regions having the same IED source, we designed and optimized an iterative algorithm using additional spatial coordinates of all intracranial EEG recording contacts. Forty-four patients with focal cortical dysplasia (FCD), who underwent epilepsy surgery, were analyzed to find criteria for maximal reduction in the number of sub-regions while maintaining clinically relevant IZ decomposition. The optimal Euclidian distance between any IED sources that result in the merging of sub-regions was found at R = 10.6 mm. The approach used significantly reduced the number of the subregions to six at the median (p=0.004, U-test) with a lower mutual overlap of 10.7 % (p<0.002, U-test). The presented method reduces the sub-regional fragmentation of IZ and maintains a detailed description of the epileptic network. It facilitates the advance of detailed analysis of the interaction between epileptic focus, IED generators, and the effect of their removal by surgery in clinical practice.
