Bilateral patellar tendon tears after a fall from standing position: Do you mind systemic diseases and medications?
Original language description
The article describes a case of bilateral patellar tendon rupture after a minor trauma verified by performing magnetic resonance imaging. However, we miss the information about the medical history of the patient because bilateral quadriceps tendon ruptures are highly correlated with systemic disease.2 We recently published a case of bilateral rectus femoris muscle rupture associated with statin treatment.3 Statins (3-hydroxy-3-methylglutaryl-coenzyme A reductase inhibitors) are commonly used for the prevention and treatment of atherosclerotic vascular disease. Apart from others (nervous, gastrointestinal, etc), one important adverse effect is related to muscle toxicity, which can manifest as tendinitis, tendinopathy, tendon rupture, muscle pain, myositis, myopathy, or rhabdomyolysis.4 Tendon rupture associated with statin therapy may be related to inhibitory effects of statins on matrix metalloproteinase activity as well as reduction and disorganization of collagen type I in tendons.5,6 As such, we suggest that physicians should be aware of relevant systemic diseases and/or medications while they question the background of (particularly bilateral) tendon ruptures.
