    Analysis of Accidental Exposures in the Czech Republic for the time period of 2005-2011

    Accidental exposures and near misses are periodically summarized by the State Office for Nuclear Safety (SONS) in collaboration with the National Radiation Protection Institute. The classification, recording and reporting of accidental exposures are described in the recommendation published by SONS in 1999, amended in 2008. An analysis of 340 reports for the time period of 2008-2011 and 160 reports for the time period of 2005-2008 is presented. Attention was paid to mistakes leading to accidental exposures, to root and contributing causes of mistakes, to the phase where mistakes occurred, and to the preventive measures. The most frequent causes of accidental exposures were inattention, inadequate supervision, non-standard procedures, stressful situations, and also inadequate communication, reading of documentation, and knowledge or experience.
