Background: Coronary vasospasm is a recognized complication of pulsed-field ablation (PFA), though most evidence relates to large-footprint systems. Data on focal PFA, particularly when delivered via conventional catheters, remain limited. This study reports observations of coronary spasm associated with focal PFA using the CENTAURI system.Methods: Patients undergoing ventricular arrhythmia (VA) ablation with focal PFA between May 2023 and June 2025 were included. Procedures were guided by electroanatomical mapping and intracardiac echocardiography. A 3.5-mm irrigated-tip catheter was used for mapping and ablation. PFA applications (25 A, 3.4 ms pulse width) were delivered up to three times per site. Coronary angiography was performed before and after each application to assess vasospasm. The distance between the catheter tip and coronary arteries was measured using fluoroscopy or intracardiac imaging.Results: Thirteen patients underwent focal PFA near coronary arteries. Coronary vasospasm occurred in 4 patients (31%). Most ablation sites were located in the left ventricular outflow tract, frequently involving the great cardiac vein. The mean catheter-to-artery distance was significantly shorter in cases with vasospasm compared to those without (0.75 ± 0.65 mm vs. 3.8 ± 2.5 mm).Conclusions: Coronary vasospasm may occur in a substantial proportion of patients undergoing focal PFA, particularly when ablation is performed in close proximity to coronary arteries. These findings highlight the importance of careful spatial assessment and coronary monitoring during focal PFA procedures.
