Between December 1993 and July 2002 sixteen patients with chordoma and two patients with chondrosarcoma of the scull base were treated in our department in the Na Homolce Hospital. In this group of 18 focally irradiated patients ( 11 women,7 men) the age varied from 25 to 77 ( median 42,5) years. A minimal dose with a margin of 11- 27,5 ( median 18 Gy) was applied to the tumor with median volume of 11,8 cm3 . The follow- up was carried out 22- 204 months ( median 107 months) . Nine patients showed clinical improvement, seven worsened clinically and two patients died in connection with continuing tumor growth. Post- irradiation follow up imaging showed a reduction in tumor size in 3 patients ( 17%), no further tumor growth in 3 patients ( 17%) and tumor growth progression in 12 patients ( 66%). In the post- irradiation period in this group of 18 patients five patients underwent repeated open resection, four patients underwent re- irradiation, in two patients because of growth progression of the irradiated it was only possible to apply symptomatic treatment. Chordomas and chondrosarcomas are invasive tumors of locally aggressive behavior demanding in most cases a combination of both surgical and radiation treatment.
