    Cereal prolamins and their role in the diagnosis of celiac disease

    Celiac disease (celiac sprue, gluten enteropathy) is an autoimmune disease caused by intolerance to gliadins and other prolamins in certain cereals in individuals with a genetic predisposition (HLA-DQ2 and HLA-DQ8). Global prevalence has been rising in recent years to about 1.4% and has increased more than 4 times in the last 50 years (Rubio-Tapia et al. 2009; Singh et al. 2018). Intake of gluten leads to the development of an autoimmune reaction and damage of the small intestine mucosa epithelium, which loses its function. The only treatment available today is a lifelong gluten-free diet based on strict exclusion of gluten presented in cereals like wheat, rye, or barley but also oat. The role of oats in the pathogenesis of celiac disease is widely discussed by experts because oats cause immune system response only in some individuals. As cereals are an important source of fiber and some vitamins, it is desirable not to omit them in patients in whom it is possible.
