Original language description
Flow cytometric quantification of the major populations of peripheral blood lymphocytes is key in the diagnostic screening of inborn errors of immunity (IEIs). However, no consensus exists about the level of detail required at the lymphocyte population level. Analysis of total, CD4 and CD8 PB T-cell levels, and total B- and natural killer (NK) cell levels through combined assessment of CD3, CD4, CD8, CD19, and CD56 is broadly recommended for the screening of severe combined immunodeficiency (SCID), nonsevere combined immunodeficiency (CID), predominantly antibody deficiency (PAD), and diseases of immune dysregulation (DIDs), and it is used at many reference centers.4 However, several guidelines suggest that more in-depth dissection of the maturation compartments (eg, naive and memory) of TCD4, TCD8, and B cells (eg, immunoglobulin heavy chain [IgH]-unswitched vs IgH-switched memory B cells [MBCs] and plasma cells [PCs]) is needed for more sensitive and accurate IEI diagnosis.
