A 74-year-old woman with a history of hypothyroidism treated with levothyroxin and no history of vascular risk factors presented with acute onset left hemiparesis [National Institute of Health Stroke Scale score (NIHSSscore): 5 points]. She underwent a brain MRI showing multiple, bilateral acute ischemic lesions located both in the anterior and posterior circulation that were indicative of a potential embolic source. Treatment with heparin was therefore introduced but the next day the patient had a severe deterioration of her neurological condition. A second brain MRI at the seventh day of ictus showed no evidence of intracranial bleeding but multiple new acute ischemic lesions. More specifically, there was no evidence of cerebral microbleeds on T2* sequences.
