Low density receptor-related protein 4 (LRP4), has been recognized recently as a novel antigen implicated in the pathogenesis of myasthenia gravis (MG).1-4 Patients who harbor antibodies directed to LRP4 comprise a distinct subgroup of seronegative MG patients whose clinical phenotype has not yet been characterized well. Among the 4 available studies, the percentage of LRP4 positive patients within the seronegative group varied from 2% to 46%.1
