Language impairments are common yet often underrecognized in patients with Lewy body diseases (LBDs), even at the stage of mild cognitive impairment (MCI-LB). This study aimed to identify specific deficits in the comprehension of syntactically complex sentences in MCI-LB patients and to determine their neural correlates within dorsal and ventral language pathways using functional connectivity analysis. Twenty-five MCI-LB patients and 25 healthy controls (HC) underwent task-based fMRI during the Test of Sentence Comprehension (ToSC). Functional connectivity between predefined regions of interest in dorsal and ventral language streams was assessed using psychophysiological interaction (PPI) analysis with FDR-corrected statistical thresholds. MCI-LB patients showed significantly lower ToSC performance compared to HC. Connectivity analyses revealed reduced functional coupling predominantly within the dorsal language pathway during processing of noncanonical syntactic structures. Specifically, decreased connectivity was observed between the anterior superior temporal gyrus and frontal language regions in MCI-LB patients relative to controls. These findings indicate that impaired sentence comprehension in MCI-LB is associated with disrupted dorsal language network connectivity. Alterations in frontotemporal connectivity may represent a potential neuroimaging marker of cognitive decline in LBDs and contribute to understanding language dysfunction in synucleinopathies.
