The study evaluated the accuracy of preoperative tumour size assessment in patients with early-stage cervical cancer using data from the SENTIX trial, a prospective international multicentre study investigating sentinel lymph node biopsy without pelvic lymph node dissection. A total of 680 patients were analysed, comparing tumour measurements obtained by imaging with final histopathology. Postoperative stage upgrading occurred in 27.5% of patients and downgrading in 10.9%. A discrepancy of ≥10 mm between imaging and pathology was identified in 22.8% of cases, resulting in both insufficiently radical and excessively radical surgical procedures. The findings demonstrate that inaccurate tumour measurement remains a significant contributor to incorrect staging and suboptimal surgical planning. The study highlights the need for international guidelines to define acceptable deviations in tumour size assessment and criteria for adjuvant treatment in patients receiving insufficient surgical radicality.
