Renal biopsy is considered a golden standard to diagnose vasculitis and assess disease activity/chronicity stage in patients with renal involvement in ANCA-associated vasculitis (AAV). The aim of this study was to assess the potential value of a biomarker panel for a non-invasive assessment of renal disease in AAV. The cohort consisted of 48 patients with AAV (M/F 29/19, median age years [range 28-81], PR3-ANCA/MPO-ANCA/double PR3- and MPO-ANCA 22/24/2) sampled at the time of their renal biopsy and 10 healthy controls. We measured PRO-C6 (formation of collagen type VI), DKK-3 (Dickkopf-related protein 3), C3M (degradation of collagen type III), soluble CD163 and EGF (epidermal growth factor) in paired serum and urine samples using ELISA method along with routine measurements of creatinine. Urinary markers were all normalized to urinary creatinine. Kidney biopsies were evaluated with a scoring system for AAV and interstitial fibrosis was quantified according to the Banff schema: grade 0: fibrosis less than 5%, grade 1: mild fibrosis (6-25%); grade 2: moderate fibrosis (26-50%) and grade 3: severe fibrosis (more than 50%).
