Azathioprine (AZA) and 6-mercaptopurine (6-MP) are effective therapeutic modalities in treatment of IBD. However, about 20-35% of patients do not respond and about 15% of patients develop intolerance to AZA/6-MP. The levels of erythrocyte 6-MP metabolites show a strong inverse correlation between 6-thioguanine (6-TG) levels and disease activity. A wide range of erythrocyte 6-TG levels was associated with clinical response to therapy, patients with 6-TG levels > 250 pmol/8 x 10 8 red blood cells were uniformly asymptomatic. 6-thioguanine therapy is an alternative to AZA/6- MP in children with acute lymphoblastic leukemia, which gives rise to high 6-TG nucleotides levels-the most active metabolites of AZA/6-MP. The direct use of 6-TG may offer arapid andsafer alternative, especially in selected patients failing or intolerant to azathioprine or 6-mercaptopurine. 6-TG is related to azathioprine and 6-mercaptopurine and interferes with guanine synthesis by competing for hypoxanthine-guanin
