    Aim: We reported that autologous intracoronary mononuclear bone marrow cell (BMC) transplantation improves regional myocardial function of the infarcted wall in a dose-dependent manner (Am Heart J 2006; 152: 975. e9-15.). The aim ofthis study was to analyse perfusion and metabolism in patients with and without the Jeft ventricular (LV) function improvement due to high cell dose transplantation. lVIethods: We included 62 consecutive patients with irreversible damage after the first acute STEM!. Thirty-one paticnts were treated using BMC transplantation, while 31 patients were randomly integrated into a control group. The gated SPECT LV ejection fraction (LVEF) and end-diastolíc/ end-systolic volumes (EDV/ESV) were quantified before BMC therapy and then3 months later. Results: In the control group, the baseIine mean LVEF of 41?9% increased to 44?9% (P=NS); wc observed worsening of EDV/ESV from l54?36 ml/93?32 ml to 167?47 ml/96?4l ml.     Cíl: V předchozí práci jsme referovali, že při léčbě intrakoronární transplantací buněk kostní dřeně závisí regionální úprava funkce levé komory na dávce podaných buněk (Am Heart J 2006; 152: 975.e9-15.). Cílem této práce byla analýza perfuze a metabolismu u pacientů s a bez úpravy funkce levé komory vlivem transplantace vyšší davky buněk.
