    Incidence of emergence delirium in the PACU: prospective observational trial

    Background and Goal of Study: Emergence delirium (ED) is a serious postoperative complication in paediatric anaesthesia. It is characterised by psychomotor and perception disorder and has negative impact on morbidity and mortality. The aim of this trial was to determine the incidence of ED in paediatric patients after general anaesthesia, using PAED (Paediatric Anaesthesia Emergence Delirium) score in the PACU (post-anaesthesia care unit). Materials and Methods: The trial was approved by the Ethics Committee by the University hospital Brno (date of approval 09/09/2020), registered on clinicaltrials.gov (ID: NCT04531020) and designed as prospective observational trial. The trial is ongoing in from 09/2020 until 06/2021. PAED score, WATCHA score and Richmond agitation and sedation scale (RASS) were measured in 0., 5., 10., 15., and 20. minute after PACU admission and after obtaining RASS GREATER-THAN OR EQUAL TO - 2 (the first eye contact with the caregiver). ED was (according to recommendation) defined as PAED score &gt;10 points. In case ED developed, the duration of ED, the number of therapeutic interventions and the cumulative dose of administered sedatives was recorded. The average PAED, WATCHA, RASS scale, the incidence of postoperative nausea and vomiting, the cumulative dose of administered antiemetics, the type of anaesthesia induction (inhalation, intravenous), type of anaesthesia (TIVA, combined, inhalational), length of the surgery, type of the surgery was reported. Results and Discussion: We present preliminary data from the first 6 months of data collection. In sum 858 patients (09/2020-02/2021) were included, 752 patients were included in the preliminary data analysis. The overall incidence of ED was 87.23 %. The incidence of ED measured after obtaining RASS GREATER-THAN OR EQUAL TO - 2 was 34.97 %. The overall average PAED score was 8.24 points. The average PAED score measured after obtaining RASS GREATER-THAN OR EQUAL TO - 2 was 7.75. Conclusions: According to the preliminary data the overall incidence of ED in the PACU was 87.23 %. The incidence of ED measured after the first eye contact with the caregiver (obtaining RASS GREATER-THAN OR EQUAL TO - 2) was 34.97 %.
