Repairing Epileptic Sleep with Non-Invasive Temporal Interference Brain Stimulation

Patients with drug-resistant epilepsy often experience severely disrupted sleep architecture, particularly a reduction in restorative NREM3 sleep due to nighttime epileptiform discharges. This fragmentation not only exacerbates seizure burden but also impairs memory consolidation and worsens quality of life. While current neuromodulation therapies such as deep brain stimulation or vagus nerve stimulation can offer benefits, they are invasive, costly, and limited to a narrow subset of patients. This project proposes a novel, non-invasive neuromodulation approach using temporal interference (TI) electrical stimulation during sleep to modulate deep brain structures such as the thalamus and hippocampus. TI uses two high-frequency currents that intersect to produce a low-frequency envelope at depth, enabling targeted brain modulation without affecting superficial cortex. Our hypothesis is that by enhancing NREM3 sleep, we can indirectly suppress epileptiform activity and improve cognitive performance. The study will be conducted at FNUSA (Czech Republic) in collaboration with Duke University (USA). Patients undergoing clinical EEG monitoring will receive overnight TI stimulation for four consecutive nights. Sleep architecture and epileptic biomarkers will be evaluated through polysomnography and EEG recordings. Pre- and post-stimulation neurocognitive tests and subjective sleep questionnaires will be used to assess clinical impact. In addition to evaluating the effects of stimulation, the project includes personalized modeling of current distribution using CT and MR imaging. The project is expected to yield a validated protocol for non-invasive nighttime brain stimulation in epilepsy, with broader implications for neurological and psychiatric conditions affected by sleep. Results will be disseminated through peer-reviewed publications, data sharing, and integration into future clinical trials.
