rTMS-enhanced psychotherapy for borderline personality disorder

Borderline personality disorder (BPD) is a devastating pervasive mental disorder characterized by emotion regulation impairment and marked impulsive behavior, including self-harming and suicidal behavior. 40 % to 85 % of patients with BPD attempt to commit suicide in their lifetime, and up to 10% of patients with BPD actually do commit suicide, making BPD a highly lethal condition. Psychotherapies targeted specifically at the treatment of BPD have been developed, among which Dialectical behavior therapy (DBT) is one of the most prominent and effective. However, average dropout rates in DBT are about 28% which constitutes one off the main problems in psychotherapies for BPD. Among other treatment possibilities in BPD, repetitive transcranial magnetic stimulation (rTMS) has been repeatedly used in recent years. However, rTMS effects tend to decrease after the intervention within a range of weeks to month. In the current highly innovative project, we aim to overcome the biggest problems of both DBT (high dropout rates) and rTMS (short effect duration) by combining both treatments. 60 patients with borderline personality disorder will undergo 3 weeks of prefrontal active or sham rTMS, and half of the patients will additionally undergo 24 weeks of DBT. Behavioral and neuroimaging effects of the treatments will be assessed in multiple timepoints. The project is one of the first experimental trials examining the potential of rTMS augmented psychotherapy at all and it will also provide a possibly effective and short intervention to the patients at waitlist for DBT. We will also assess the neural effects of prefrontal rTMS and DBT in BP, measurement in multiple time points will allow us to examine the predictors and stability of the DBT and rTMS effects. Thus, our study will provide multiple unique contributions to the current knowledge about the treatments of BPD and can substantially enhance our understanding and promote the treatment options for BPD.
