Pathologies of the uterus and surrounding pelvic organs belong to increasingly important limitations of female infertility. This trend goes hand in hand with changing reproductive behavior of women that often postpone their pregnancies into the fourth decennium. The increase in frequency of uterine adenomyosis (AD), uterine fibroids (UF) and deep endometriosis (DE) can be observed especially in women investigated for infertility. Their incidence reaches almost 40% in such population of women and thus creates a fundamental medical, psycho-social and economic problem of our time. The correct therapeutical algorithm of such numerous cohort of patients has not been clearly defined yet. Existing literature offers only indirect evidence of worse reproductive outcomes of these women coming from retrospective or observational studies, often with the lack of comparable control group. Furthermore the effect of fertility saving surgical procedures of these pathologies on implantation potential of the uterine cavity and its possible increase after the surgery is completely unexplored. In our project we aim to define molecular-biological markers of the endometrial receptivity, their relationship to the mentioned gynaecological diseases and the effect of surgical treatment on their change. We will use the methods of high-throughput sequencing (HTS) of endometrial RNA at the tissue level as well as at the cell level. We will compare the reproductive outcomes of the surgical and conservative therapy among infertile women with AD and UF and determine the optimal course of their treatment. Our project will offer clear and comprehensive idea on characteristics of the endometrium and its parameters important for successful conception. It will provide a basis for follow up research and will help to define and optimize the treatment strategy for large group of women stigmatized with infertility.
