Development and validation of a novel metrics of metabolic acidosis: the whole-body base excess
Anotace anglicky
This project aims to develop a novel acid-base metric, whole-body base excess (WBE), to improve the characterization of in-vivo metabolic acidosis. Physicians currently rely on standard base excess (SBE), but its accuracy is increasingly criticized. Studies in healthy subjects and animals have questioned its ability to distinguish metabolic from respiratory acidosis and its agreement with actual acid excess targeted by alkalizing agents. These limitations compromise diagnosis and treatment. Building on our published theoretical work and analyses of existing experimental data, we hypothesize that SBE’s inaccuracy stems from neglecting intracellular buffering (mainly muscle and bone) and focusing solely on extracellular fluid. In modern medicine, where aging, chronic disease, and intensive care treatments lead to significant body composition variability (osteoporosis, cachexia, obesity), these inaccuracies may be further magnified. To test this, we will study metabolic acidosis in pigs induced by strong acid infusion or nephrectomy, assessing buffering in muscle, fat, bone, plasma, and interstitial fluid via biopsies and tracer dilution. We will compare WBE’s stability with SBE in pigs undergoing respiratory acidosis, as a reliable metabolic acidosis metric should remain stable under respiratory acidosis. In parallel, we will validate WBE in humans with varying body compositions—healthy individuals with different muscle mass, obese and osteoporotic patients—using bioelectrical impedance or imaging, respectively. We anticipate that WBE will correct SBE’s inaccuracies in animals and healthy subjects while requiring patient-specific adjustments in those with altered body composition. In the era of personalized medicine, WBE has the potential to change metabolic acidosis diagnosis and treatment.
