Optimizing Rectal Cancer Therapy and Organ Preservation Strategies through Advanced ctDNA and Microbiome Profiling for Precise Monitoring and Response Evaluation
Anotace anglicky
The disappearance of rectal cancer after neoadjuvant therapy, known as a clinical complete response, has become increasingly common with the advent of immunotherapy and total neoadjuvant therapy. In these cases, tumors become undetectable through imaging, endoscopy, or clinical examination, potentially eliminating the need for radical rectal surgery, a procedure associated with significant risks and a profound impact on quality of life. Instead, patients may be managed through active surveillance using the “Watch and Wait” strategy. However, current detection methods have limited accuracy, with a false-negative rate of 25–30%, increasing the risk of undetected tumor regrowth and worse oncological outcomes. This project aims to improve monitoring of treatment response and residual disease detection by integrating circulating tumor DNA analysis and microbiome profiling with artificial intelligence-powered predictive modeling. Targeted DNA sequencing, a highly standardized technique, enables the detection of specific genetic alterations linked to cancer progression, providing valuable insights into drug resistance and treatment efficacy. Meanwhile, microbiome profiling investigates the gut microbiome’s influence on cancer development, treatment response, and prognosis. The gut microbiota can shape responses to neoadjuvant therapies by modulating inflammation, immune activity, and metabolism, while these treatments, in turn, alter microbial composition. By combining these approaches with AI-driven modeling, this study will generate personalized recurrence risk profiles, offering a minimally invasive system for real-time tumor monitoring. This advanced strategy can be seamlessly integrated into clinical practice, paving the way for more personalized treatment decisions and an improved quality of life for rectal cancer patients.
