Leiomyomas are among the most common benign diseases of myometrium. Preoperative differentiation of leiomyomas from sarcomas and other pathologies is challenging, often resulting in unnecessary hysterectomies, insufficiently radical procedures or delayed treatment. Given the aggressiveness of sarcomas and specifics of usually preferred mini-invasive approach (risk of tumor dissemination during laparoscopic resection and morcellation), obtaining diagnosis prior to definitive surgery is crucial and timely prediction of malignancy is then of utmost importance. The Tru-cut biopsy (TCB) reliability for the differential diagnosis of uterine sarcoma in "myoma-like" uterine lesions has so far been verified by only a minimum of studies. MYSTR study is designed as a single arm, prospective, multicentric cohort study, that aims to evaluate the TCB of sonographically atypical myometrial lesions as a complementary diagnostic method in the preoperative management of patients scheduled for uterus preserving procedure or hysterectomy. Additionally, array-comparative genomic hybridization (aCGH) will be performed and analyzed to explore the genetic profiling potential for further diagnostic adjustment. A total of 363 patients is planned to be enrolled, accounting for an estimated 15% dropout rate. Our pilot data (n=39) demonstrated TCB sensitivity and specificity of 88.9% and 92.9%, respectively. The study will assess whether TCB, integrated with molecular profiling, improves preoperative risk stratification and influences surgical decision-making. The findings should lead to a novel diagnostic algorithm and enhance personalized surgical management for atypical myometrial lesions, potentially reducing unnecessary hysterectomies and improving patients’ outcomes. Therefore, it would be a great mistake not to thoroughly investigate this step between the diagnostic and surgical phases of myometrial lesions management and potentially include it in the optimal treatment algorithm.
