Modern immunotherapeutic agents have revolutionized the treatment landscape of multiple myeloma over the past decade, with the most significant advancements observed in chimeric antigen receptor (CAR) T-cell therapy and bispecific T-cell engagers (TCEs). Despite their remarkable efficacy, both primary and acquired resistance remain challenges. Until now, we do not have a tool to select optimal patient and optimal immunotherapeutic drug for specific subgroup of multiple myeloma. Emerging research highlights the critical role of endogenous T-cell fitness and phenotype in determining the efficacy of CAR T-cell and TCE therapies. In contrast, very limited works have investigated the impact and role of other immune cell populations (e.g. myeloid immunosuppressive cells) and mediators of immune reactions such as cytokines or chemokines. During treatment, immunotherapeutic agents can drive the selection of exhausted T-cell subsets and mutated malignant plasma cells, contributing to treatment resistance. Current insights are constrained by small patient cohorts, primarily treated with BCMA-targeted therapies, and studies that focus on either immune effectors or malignant plasma cells, , thus failing to comprehensively capture the intricate immunotherapy-induced adaptations. The originality and innovativeness of our project emerge from longitudinal monitoring of various immune cells and cytokines in combination with detection of genomic/ transcriptomic aberrations before and after treatment. Based on this approach, we aim to identify critical predictive markers for treatment response and define particular mechanism of resistance linked to specific immunotherapeutic agents. To accomplish these goals, we will combine single-cell transcriptomics, flow cytometry methods along with multiplex immunoassay all of which will be run on samples from patients treated with agents targeting different antigens.
