Obstructive Sleep Apnea as a Potential Biomarker of a Specific Phenotype of Parkinson’s Disease

Parkinson’s disease (PD) is a neurodegenerative disorder classified among alpha-synucleinopathies, exhibiting diverse clinical phenotypes with varying symptoms and prognoses. The biological or clinical factors driving this variability remain largely unknown. One potential contributor is co-pathology with Alzheimer’s-related changes in the central nervous system (CNS). Obstructive sleep apnea (OSA), a sleep-related breathing disorder causing recurrent airway obstruction, nocturnal hypoxemia, and sleep fragmentation, has been linked to an increased risk of cognitive impairment and Alzheimer’s-type dementia. A connection between OSA and PD has also been observed, and our pilot study suggested an association between OSA and early bilateral hippocampal atrophy in PD. This project aims to expand on these findings by evaluating a larger cohort, including both early and advanced PD stages. All participants will undergo polysomnography, and OSA-positive cases will receive continuous positive airway pressure therapy. A longitudinal analysis will assess how OSA and its treatment influence motor and cognitive decline, brain atrophy, and other MRI parameters. Using blood and cerebrospinal fluid biomarkers of Alzheimer’s pathology and amyloid PET imaging, we will explore whether OSA in PD contributes to Alzheimer’s-like brain changes. Our findings may clarify the role of OSA in PD progression and determine whether it is a modifiable risk factor. Identifying concurrent neurodegeneration in PD could have important implications for future biological therapies targeting amyloid deposits.
