    The DART project (Differentiation patterns, clinical Associations, and Rejuvenation of T-cell Immunity in Primary ImmunoDeficiencies) focuses on analyzing adaptive immunity in patients with defects of thymic development (DTD) and DNA repair defects (DRD) leading to chromosomal instability. Both groups suffer from T-cell lymphopenia, but due to different causes — DTD result from impaired function of the thymic stroma, whereas DRD are caused by defective DNA repair mechanisms in T cells. Thanks to the early diagnosis of primary immunodeficiencies in the neonatal screening center at the Department of Immunology, University Hospital in Motol, we will investigate the relationship between clinical progression and age-related differentiation of T cells while comparing patients with DTD and DRD. Using single-cell -omics analyses (e.g., CITE-seq) and bioinformatics approaches, we will map and contrast the distinct T-cell developmental trajectories. Microbiome profiling together with the phenotypic and transcriptomic characterization of mucosal T cells from upper airways will provide modern multi-omic datasets to inform on the connection between impaired T-cell function and recurrent respiratory infections. Therapeutic options for patients with mild to moderate T-cell lymphopenia remain limited. In collaboration with the Laboratory of Adaptive Immunity at IMG CAS, we will test compounds that reduce T-cell exhaustion and senescence (metformin, thymosin alpha 1, and nicotinamide riboside) on mouse models (e.g., 22q11.2 deletion syndrome) and patient-derived cell cultures and organoids. Some DRD patients are already using these compounds to alleviate neurological symptoms in clinical practice. This project integrates clinical and academic research, leverages unique cohorts from tertiary and screening centers, and employs cutting-edge methodologies to identify novel therapeutic approaches.
