Molecular tools for outcome prediction of kidney transplantation from marginal donors
Anotace anglicky
The increasing proportion of kidneys procured from deceased donors that are subsequently discarded for transplantation represents a significant medical problem. Identifying kidneys from older donors that are suitable for transplantation because they will achieve acceptable long-term outcomes can increase the number of donors, which is the unmet medical need in organ transplantation. Whole-genome transcriptomic assessment of donor biopsies together with machine learning methods allows for more precise donor stratification beyond traditional clinical metrics. Our study, together with other research, has shown that some kidneys transplanted from older donors or donors with a high risk index (KDRI) may still have acceptable function in the first year. Within the framework of the presented grant, the applicability of the already created model will be verified for identifying kidneys with a better prognosis only in the group of expanded criteria donors (ECD) and, newly, in donors after circulatory death (DCD), where organs are exposed to warm ischemia. Gene expression in ECD (n=130) and DCD (n=60) will be examined using microarray. Data from existing external cohorts (n=308) will be used to validate the model. Subsequently, we will characterize the transcriptome evolution over time from hour zero biopsy to 3 months after kidney transplantation (n=340) to improve the model's ability to predict rapid fibrosis progression. The model will be refined by increasing the dataset size and by using newly available machine learning techniques (prototype contrast learning). The created model will allow for a more accurate estimation of graft function (eGFR) after transplantation, which is essential in the decision on immunosuppressive treatment, particularly when adjustments are needed due to inadequate graft function.
