Correcting disturbances of the blood flow through the fetal side of the placenta caused by certain antidepressants and smoking of conventional and electronic cigarettes
Anotace anglicky
The project addresses reduced blood flow through the fetal side of the placenta as a pathogenetic factor of impaired fetal growth in mothers who smoke conventional or electronic cigarettes or use antidepressants from the selective serotonin reuptake inhibitors (SSRIs) group, and the possibilities of causal therapeutic intervention. Adequate fetoplacental perfusion is essential for prenatal development, with impairment leading to fetal growth restriction with neonatal and lifelong health risks. In addition to the known influences limiting fetoplacental perfusion, e.g. chronic hypoxia or maternal alcoholism, other factors known to limit fetal growth, such as smoking or SSRI use, may act similarly. The project’s first goal is to prove this mechanism, and the ultimate aim is to test possible improvement of perfusion in these situations. For smoking, we will test antioxidants and AT1 blockers as potential therapies and assess whether e-cigarettes are safer than conventional ones in this respect. For SSRIs, we will test the hypothesis that the effects on the fetoplacental vasculature are mediated by different types of serotonin receptors than those described for antidepressant action, and thus selective inhibition of 5HT2A and 5HT1B receptors will improve fetoplacental perfusion without risking interference with the antidepressant effect. Given the known role of macrophage polarization in various vascular disorders, we will determine how is the polarization of fetoplacental macrophages, the Hofbauer cells, altered by smoking and SSRIs. We will use ex vivo perfused placentas from healthy newborns of mothers who regularly use electronic or conventional cigarettes or SSRIs. We will measure vascular resistance on the fetal side,vasoreactivity and the effect of administration of an antioxidant (tempol), an AT1 blocker (losartan) and 5HT2A and 5HT1B receptor inhibitors (ritanserin and GR127935). We will determine the polarization of Hofbauer cells by flow cytometry.
