Atherosclerotic cardiovascular disease (ASCVD) has well-known known risk factors (smoking, dyslipidaemia, hypertension, obesity and diabetes), but these factors do not fully explain the high prevalence of ASCVD. Both DNA variability and recently discussed epigenetic changes are important factors in the development of ASCVD. Epigenetic information is not encoded in the DNA sequence and is subject to change throughout life. Epigenetic processes are "rapid" responses to environmental changes (physical activity, dietary habits, and environmental pollution) that primarily affect gene expression. The main epigenetic modifier is DNA methylation. Traditional ASCVD risk factors may influence DNA methylation and ASCVD risk, and "epigenetic age" is known to be associated with ASCVD. Considerable effort has been devoted to polygenic risk assessment (genetic risk score, GRS) for ASCVD, but none has focused exclusively on DNA methylation-related polymorphisms. Other epigenetic factors are telomeres which contribute to DNA integrity and stability. The association between relative telomere length (RTL) and ASCVD has been studied, but there are still many controversies and the role of RTL in ASCVD needs further analysis. The effect of DNA methylation, telomere length and GRS generated from epigenetically associated SNPs on ASCVD risk will be analysed in patients (including women) younger than 65 years with a diagnosis of 1st myocardial infarction. We will perform molecular genetic testing to elucidate the mechanisms by which epigenetics modifies ASCVD risk. An innovative methodological approach, analysis with progressive methylation microarrays, will be used. The unique combination of screening for three genetic traits (DNA methylation status, telomere length and GRS) and the inclusion of women, who are under-represented in most epigenetic studies, makes the study internationally competitive. The expected results are in line with the stated priorities.
