Anotace anglicky

Pressure ulcers/injuries (PUs/PIs) including Medical Device-Related Pressure Ulcers (MDRPU), Incontinence Associated Dermatitis (IAD), Skin Tears, and Medical Adhesive Related Skin Injuries (MARSI) present clinical and economic challenges in the healthcare, with consequences of misrecognition and suboptimal treatment. Despite available guidelines and classification systems (e.g. EPUAP, ISTAP, GLOBIAD), diagnostic inaccuracies persist. This results in inappropriate interventions, longer healing times and increased costs. Project objective: to increase the ability of non-medical healthcare professionals (NHCPs) in the differential diagnosis of "look alike" selected skin and mucosal lesions and to optimize care by developing care algorithms. The project follows a "back to basics" approach: strengthening core clinical competencies in wound assessment, especially for the NHCPs. Methodology: prospective observational study in acute inpatient care, residential social care and home care, with a target population of at least 200 wounds. The study includes three phases: initial assessment of current diagnostic practices and identification of knowledge gaps, development of standardised decision tree-based diagnostic algorithms and clinical treatment algorithms (pathways) through expert consensus and Delphi method and a comprehensive educational intervention coupled with a 6month pilot implementation in a centralized digital documentation platform. Expected outcomes: refinement of classification and differential diagnosis, streamlining of care pathways and ensuring consistent and quality treatment of decubitus and other skin and mucosal injuries (similar and easily interchangeable) associated with nursing care and risk of non-healing wounds. Multidisciplinary collaboration between academics, clinicians and IT specialists will bridge the gap between theoretical recommendations and practice and improve prevention and treatment.
