    Integrated National Surveillance of Antifungal Resistance in Candida: Clinical and Environmental Perspectives in the Czech Republic

    Bloodstream infections caused by yeasts and yeast-like micromycetes are a common complication among hospitalized patients, particularly those critically ill. Over the past decade, the spectrum of Candida species that cause these infections and their susceptibility profiles to antifungal agents have changed significantly due to new therapeutic approaches. The project aims to establish a long-term monitoring platform for the prevalence of Candida pathogens and their susceptibility to antifungal agents (EUCAST reference methodology ) in healthcare facilities in the Czech Republic (CR). The goal is to create an electronic surveillance database that serves as a tool for monitoring the diversity of Candida species in the patient population in the CR and trends in the development of acquired resistance. The molecular-genetic basis of phenotypically proven resistances will also be determined. Another critical aspect of the project is to investigate the presence of resistant Candida strains in the wastewater of healthcare facilities. The aim is to explore how the use of antifungal agents entering wastewater contributes to the development of resistance and its potential to further spread in the environment and healthcare facilities. This approach involves monitoring prospective monitoring of azole antifungals in wastewater along with resistant pathogens, which may provide crucial pilot data for the development of a methodology for nationwide surveillance in the CR. The project is essential for a better understanding of the epidemiology of yeast infections and mechanisms of resistance, which is critical for effective prevention and treatment of these infections. This systematic approach will allow for early identification and response to the spread of resistant pathogens, thus contributing to the protection of public health.
