Artificial Intelligence-Driven Multimodal Analysis of Speech Emotions for Early Stage Assessment of Basal Ganglia Disorders

Emotional processing impairments represent one of the earliest and most affected indicators of basal ganglia disorders, specifically Parkinson's disease (PD) and Huntington's disease (HD). Signs of emotional abnormalities can appear up to 10 years before the onset of cardinal motor manifestations and generally worsen as the disease progresses, contributing to social isolation and degradation of interpersonal interactions. With disease-modifying treatments for PD and HD, which are under development, the early diagnosis of the disease onset is essential to establish the efficacy of such therapies. In this context, the assessment of emotional disruptions has unique potential. While usually assessed through questionnaires, prone to recall bias, emotional states can be identified via objective automated orofacial analysis combining acoustic, linguistic, sentiment, and video domains. Technological advances in complex multimodal neural network architectures can significantly improve the detection of emotional impairments, revealing subtle early neurodegenerative changes previously undetectable. However, the application of state-of-the-art artificial intelligence techniques for speech emotion impairment detection in early-stage PD and HD has never been studied before. Therefore, this project aims to establish and evaluate orofacial measures in spontaneous images descriptions with specific emotional content in subjects with isolated rapid eye movement sleep behaviour disorder (i.e., a case of prodromal PD), early PD, and premanifest to manifest HD. Additionally, the phenotypic effects such as sex, age, and the presence of depression will be examined. The developed methodology can be adapted for remote telehealth applications, facilitating widespread screening and monitoring of at-risk populations. The project's potential to integrate with existing digital health platforms underscores its applicability and impact on current clinical practices.
