Selective multi-receptor cannabinoid therapy for arthritic pain
Anotace anglicky
There is evidence for the involvement of endocannabinoid system in the pathogenesis of osteoarthritis (OA) and rheumatoid arthritis (RA) in preclinical in vitro and vivo data, however, its role in humans remains insufficiently explored. Both from literature and from our preliminary data it has been shown that endocannabinoid and exogenous cannabinoids act on not only on typical endocannabinoid receptors like CB1R and CB2R, but also on other types of receptors that affect pain and inflammation. Phytocannabinoids, synthetic cannabinoids and endocannabinoids exert complex and often counteracting effects on numerous receptors and targets involved in arthritic pain and inflammation. Our hypothesis is that by targeting multiple mechanisms of action and blocking unwanted effects of these compounds, the desired analgesic effect could be achieved, breaking the cycle of unsatisfactory results from cannabinoid-based therapies, which have likely been caused by the non-selectivity of the compounds. The primary goal of this project is to identify suitable cannabinoid system modulators to specifically affect selected receptors/pathways relevant to pain and inflammation in rheumatic diseases and to assess their therapeutic potential in models of OA and RA. First, we plan to conduct in vitro experiments using fibroblasts, chondrocytes, and PBMCs to test cannabinoid receptor agonists, antagonists, and enzyme inhibitors for their analgesic and anti-inflammatory properties using methods like cytokine, chemokine analysis and measuring gene expression. The second aim is to identify biomarkers in patient serum and synovial fluid samples that correlate with pain and inflammation. This involves analyzing cytokines, chemokines, and endocannabinoid levels. Finally, we aim to assess the therapeutic potential of optimal cannabinoid combinations in preclinical models of OA and RA.
