Therapierefraktäre myofasziale Schmerzen am Nacken und Schultergürtel stellen einen nicht unerheblichen Anteil am orthopädischen, unfallchirurgischen und auch neurologischen Patientengut dar. Neben der oralen/parenteralen medikamentösen Therapie, der physikalischen Therapie, der Physiotherapie, der manuellen Medizin sowie Nadelverfahren (Akupunktur, Dry Needling) und der Stoßwellentherapie stellen infiltrative Verfahren eine Hauptrolle der Behandlungsmöglichkeiten dar, ggfs. auch ergänzt um psychopharmakologische und psychotherapeutische Behandlungen. Bei den infiltrativen Verfahren finden eine Reihe von Substanzen Verwendung: Homöopathika, NaCl, verschiedene Lokalanästhetika (zum Teil in Verbindung mit Kortikosteroiden), Botulinumtoxin. In diesem Beitrag soll neben einer allgemeinen Einführung zu Wirkungsweisen und Indikationen für Botulinumtoxin diese Behandlung anhand der Literatur dargelegt und diskutiert werden, verbunden mit einem praxisorientierten Ablaufschema und Einordnung in ein Gesamttherapiekonzept.

Therapy-refractory myofascial pain on the neck and shoulder girdle represent a not inconsiderable proportion of the neurological-orthopedic patient population. In addition to procedures of oral / parenteral drug therapy, physical therapy, physiotherapy, manual medicine and needle procedures (acupuncture, dry needling), infiltrative procedures are also used The main role of the treatment options represents, possibly also supplemented by psychopharmacological and psychotherapeutic treatments. A number of substances are used in the infiltrative processes, such as various local anesthetics, possibly with the addition of corticosteroids, but also botulinum toxin and physiological saline, the latter often chosen as a placebo group in studies, but also as therapy in everyday clinical practice. In addition to a general introduction to modes of action and indications for botulinum toxin, this article is intended to explain and discuss this treatment based on the literature, combined with a practice-oriented flowchart and assignment to an overall therapy concept.
