    EUS-directed transgastric ERCP (EDGE) was first introduced in 2014 as a therapeutic option for patients with Roux-en-Y gastric bypass requiring ERCP. This procedure involves EUS-guided placement of a lumen-apposing metal stent (LAMS) to connect the gastric remnant to the gastric pouch, allowing for passage of a duodenoscope through the LAMS and performance of conventional ERCP. Since its inception, EDGE has become an internationally performed procedure, proven to be safe and efficacious. More recently, a variation of this technique has emerged: EUS-directed transgastric intervention (EDGI), in which other interventions besides ERCP are used after gastro-gastric LAMS placement. EDGI has also become a well-known, safe, and efficacious procedure with more and more new indications emerging over time. In this case, a patient who had previously undergone a Roux-en-Y gastric bypass experienced severe diarrhea, presumably from bile acid malabsorption related to his bypass, with minimal response to medical management and with significant associated morbidity. An efferent loop to gastric remnant EDGI was performed, with rapid and complete resolution of diarrhea. This case nicely illustrates the importance of thinking outside the box and of applying established procedures to novel indications in order to best help our patients.
