Cervical internal carotid artery dissection can hinder endovascular treatment due to collapse of the true lumen. We describe a bail-out technique using temporary flow reversal with a balloon guide catheter and contralateral angiography to visualize and access the true lumen. In three cases, this approach enabled successful recanalization without complications.
