There is increasing evidence that circadian rhythm abnormalities are involved in the pathophysiology of narcolepsy. The primary circadian pacemaker, the suprachiasmatic nucleus, has a strong influence on hypocretin release. Experimental data have demonstrated the existence of functional hypocretin neurons and a pineal gland circuit able to modulate melatonin production. Clinical data support altered circadian rhythm of melatonin concentration in some hypocretin-deficient patients with a potential connection to daytime sleepiness and abnormal circadian rhythm of body temperature. Clinical manifestations of the disease suggest some circadian aspects as well. Comorbidity of narcolepsy with primary circadian sleep-wake disorders is extremely rare; however, daytime sleepiness may be seen in almost all circadian sleep-wake rhythm disorders with varying severity and timing. Excessive daytime sleepiness and/or difficulties with nocturnal sleep may present differential diagnostic challenges and should be verified by objective examination: most frequently by actigraphy, if necessary, by polysomnography and multiple sleep latency test. Therapeutic approaches include behavioral and pharmaceutical procedures, while melatonin receptor agonists and dual orexin receptor agonists may represent future trends.
