The replantation of the four finger block was done for an injury on the right, dominant hand of a 29-year-old patient. After six months an arteriovenous fistula developed on the fourth finger. The other fingers were not affected. Following an examinationby digital substraction angiography, selective ligature of the communal digital artery in the fourth interdigital space was performed. Cathetrization and embolization methods were not used. In the case of AV fistula formation before the radical intervention, selective ligature of an artery causing the blood system dilatation is an advantage
