We report a unique case of giant frontal sinus pneumocele in a predisposed patient with Marfan syndrome. This pneumocele presented potentially life-threatening massive intracranial spread; caused frontal lobe compression, brain midline shift, and lateral ventrikle compression; and imitated lumboperitoneal shunt malfunction. The cause was a fungal infection in the frontal sinus, which obstructed the nasofrontal duct, acting like a unidirectional valve. Development may have been facilitated by the patient’s primary diagnosis of Marfan syndrome associated with skeletal fragility. Although frontal sinus pneumocele spread into the orbit has been described, this is the first reported case with such extensive and potentially life-threatening intracranial spread. We assume that the presence of a lumboperitoneal shunt contributed to this unique occurrence. Despite the huge pneumocele volume, an endoscopic procedure using a combined transnasal and external approach was successful and provided long-term relief.
