Right ventricular outfl ow tract obstruction caused by sinus of Valsalva aneurysm

  • Popis výsledku v původním jazyce

The article presents the case of a 66-year-old patient with progressive exertional dyspnoea who was diagnosed with right ventricular outflow tract obstruction caused by an aneurysm of the right sinus of Valsalva. Echocardiography and cardiac magnetic resonance imaging demonstrated subpulmonary obstruction associated with aneurysmal dilatation of the sinus of Valsalva measuring 47 mm in diameter. Mild aortic regurgitation, severe tricuspid regurgitation, and signs of significant resting pulmonary hypertension were also identified. The diagnosis was further confirmed by selective coronary angiography with aortography, right-sided cardiac catheterisation, and cardiac computed tomography angiography. Because of the future risk of aneurysm rupture, surgical treatment was recommended. The article highlights an extremely rare clinical presentation of a sinus of Valsalva aneurysm, in which prolapse into the right ventricular outflow tract results in dynamic obstruction of blood flow.
