This study evaluated changes in the faecal microbiome and metabolome after anti‑TNFα therapy in children with Crohn’s disease (CD) and compared them with children diagnosed with juvenile idiopathic arthritis (JIA), a condition that rarely affects the intestine. A total of 530 stool samples were analysed using 16S rRNA V4 region sequencing and ¹H NMR metabolomics. Anti‑TNFα therapy in CD led to a significant reduction in faecal calprotectin and induced specific microbial and metabolite changes that were not observed in JIA. In CD, the abundances of Blautia, Ruminococcus, Intestinibacter and Erysipelatoclostridium increased, whereas Allistipes decreased. Among metabolites, glucose and glycerol increased, while isoleucine and uracil decreased. The findings indicate that the microbiome shifts following anti‑TNFα therapy are secondary to the improvement of intestinal inflammation.
