    Is breastfeeding in MS harmful or not? An answer from real-world Czech data

    TThis study evaluates the impact of breastfeeding and its duration on the course of multiple sclerosis (MS) in Czech women, using real‑world data from the national ReMuS registry. The objectives were to identify factors associated with not initiating breastfeeding after delivery, to assess the influence of breastfeeding on disease activity, and to examine the assumption that breastfeeding is not harmful for MS patients. Using propensity score matching, we compared the Expanded Disability Status Scale (EDSS), annual relapse rate (ARR), and confirmed disease worsening (CDW) between breastfeeding and non‑breastfeeding women, including a subgroup of women who resumed disease‑modifying therapy (DMT) shortly after delivery. Analysis of 1681 deliveries from 2013 to 2020 showed no significant differences in the change of ARR, EDSS, or 6‑month CDW between the groups. Breastfeeding women did not experience worse clinical outcomes, even without initiating DMT postpartum. The findings indicate that breastfeeding in Czech women with MS does not negatively affect the disease course and can be safely supported.
