Background:Preoperative urinary infections and colonization may contribute to postoperative complications in elderly trauma patients undergoing orthopedic procedures. Despite known risks associated with urinary catheterization, limited research has examined the direct impact of preoperative urinary conditions on surgical outcomes in this population.Objectives:This study aims to assess the relationship between preoperative urinary infections or colonization and postoperative complications, including urinary infections, febrile episodes, delirium, circulatory instability, sepsis, and mortality. Secondary objectives focus on frailty correlations and the effects of antibiotic therapy.Methods:This observational study is conducted at Tomáš Baťa Regional Hospital in Zlín, Czech Republic, with a projected sample size of 50 to 100 patients. Preoperative evaluations include urine analysis, frailty scoring, symptom assessment, and antibiotic usage documentation. Postoperative complications are monitored using laboratory findings, clinical assessments, and standardized evaluation tools such as the Confusion Assessment Method for intensive care unit. Statistical analyses involve contingency tables, independence tests, and correlation coefficients, with significance set at P < .05.Anticipated results:It is anticipated that preoperative urinary infections and colonization will be associated with increased postoperative complications, particularly sepsis and delirium. Findings are expected to support refined preoperative screening protocols and improved perioperative management strategies for elderly trauma patients.Conclusion:This study aims to clarify the role of preoperative urinary conditions in postoperative morbidity and mortality, providing evidence for optimizing perioperative risk stratification. Further research with larger cohorts and long-term follow-up is needed to validate findings and refine clinical guidelines.
