The lecture presented at the Annual Congress of the Czech Neurosurgical Society and the Joint Meeting with the Polish Society of Neurosurgeons focused on the far-out syndrome associated with Bertolotti syndrome as a rare but clinically important cause of lumbosacral radicular pain. The presentation addressed pathophysiological mechanisms, clinical presentation, and diagnostic approaches, with particular emphasis on advanced imaging techniques for identifying extraforaminal nerve root compression. Attention was also given to therapeutic options, including both conservative and surgical management, and their clinical outcomes. The lecture highlighted the importance of accurate differential diagnosis in patients presenting with atypical vertebrogenic symptoms.
