The effectiveness and safety of the concept of administering whole blood (WB) has been a topic of discussion in many developed countries in recent years. Although the results of observational studies from military conflicts and civilian settings suggest that the administration of FB improves the survival of patients with polytrauma and massive bleeding, the results of prospective studies do not provide clear evidence of the benefits of FB administration in civilian settings. However, FB administration is considered safe and advantageous from a logistical point of view. In the Czech Republic (CR), the administration of PC in the form of a universal transfusion product with the official name "Whole blood, leukocyte-depleted, for universal administration" (= PKDU, SÚKL code 0007967) has been possible since 2020. Currently, the concept of whole blood administration is implemented at the University Hospital in Hradec Králové, the Emergency Medical Service of the Hradec Králové Region, the Military University Hospital in Prague, the Motol University Hospital, the Ostrava University Hospital, the Emergency Medical Service of the Moravian-Silesian Region, the Olomouc University Hospital, and the Emergency Medical Service of the Czech Army in Plzeň-Líně. According to unpublished data (Bohoněk M., Kutáč D., Řeháček V., Blahutová Š, Daluszková D. Use of Low Titre Group O Whole Blood in the Czech Republic ? abstract sent to ISBTABS24-903), approximately 800 units of PKDU were administered to 400 patients in the Czech Republic by 31 December 2022. The aim of the interdisciplinary panel was to discuss the use of PK in patients with life-threatening shortening due to trauma and to formulate a joint consensus statement on the administration of PK in the Czech healthcare system. The panel did not have the time or resources to prepare a comprehensive systematic overview of the issue.
