Summary: Intramural esophageal hematoma is a well-described but rare condition of the esophagus. This condition can develop from a variety of causes and can be easily misdiagnosed even with a known medical history, especially in the unconscious patient. We present a rare case of spontaneous intramural esophageal hematoma after recent anterior cervical fusion that was initially misdiagnosed as a cervical and mediastinal hematoma. Multiphase computed tomography, as well as upper gastrointestinal endoscopy and laboratory results, were essential for diagnosis and to avert unnecessary surgery. ? 2025, Galen s.r.o..
