Underground coal exploitation in Ostrava-Karvina region of the Upper Silesian Coal Basin is frequently limited by unsolved conflicts of interest on the surface. The terrain subsidence in mining areas results in flooding of buildings and grounds due to rising surface or groundwater. Groundwater in wells used for local water supply can be lost. The paper presents the impact of underground mining activities on hydrogeological and hydrological regime. Hydrogeology Department of Green Gas DPB, joint stock company is dealing with long-term hydrogeological monitoring from the quality and quantity viewpoint since 1990. The examples of deleterious effects of mining is presented. Numerical modeling of groundwater flow in quaternary aquifers (MODFLOW finite difference code) has been applied for predictions of terrain flooding. Several scenarios were calculated in order to optimize mining activities in relation to impacts on quaternary hydrogeology. Remedial measures were proposed (surface streams
